Q. How many friends can I refer for this program?
A. Two. You can receive up to two $50 American Express Gift Checks compliments of Mentor for referring your friends for MemoryGel® breast implant surgery.
Q. Is there a particular date that I need to have my surgery date by?
A. Yes. In order to participate in this program you must have had your surgery prior to October 1, 2010.
Q. Is there a particular date that my friend needs to have her surgery by?
A. Yes. Your friend needs to have had her MemoryGel® breast implant surgery prior to October 1, 2010.
Q. What if I have had a cosmetic procedure other than breast augmentation? Can I still refer a friend to my doctor for MemoryGel® breast implants and receive a $50 American Express Gift Check for doing so?
A. No. This program is designed to be a referral program for and about MemoryGel® implants only. Facial, body contouring or saline breast implant referrals are not included in the program
Q. What about Mentor Saline Implants? Is Mentor offering incentives for saline breast implant referrals?
A. No. This program is for Mentor MemoryGel® Breast Implant Referrals only.
Q. Does the program include a free consultation for my friend?
A. No. Providing a free consultation is your doctor's choice.
Q. Will Mentor send the American Express Gift Checks to my doctor's office?
A. No. Mentor will send the American Express Gift Checks directly to your home, using the information you provided. Sending checks to physician practices is prohibited.
The MemoryGel® Referral Program is not available to patients residing in Texas and Florida. Void where prohibited. Breast reconstruction surgeries do not qualify. Patients are encouraged to elect a surgeon of their choosing after conducting their own investigation into the particular surgeon’s training, education and experience. The referred friend must have MemoryGel® breast augmentation surgery prior to October 1, 2010. Gift checks will be processed 6-8 weeks
after Mentor receives the verified information.
Important Safety Information
Mentor MemoryGel® breast implants are indicated for breast augmentation in women who are at least 22 years old or for breast reconstruction. Breast implant surgery should not be performed in women with active infection anywhere in their body, with existing cancer or pre-cancer of their breast who have not received adequate treatment for those conditions or are pregnant or nursing.
There are risks associated with breast implant surgery. Breast implants are not lifetime devices and breast implantation is likely not a one-time surgery. You may need additional unplanned surgeries on your breasts because of complications or unacceptable cosmetic outcomes. Many of the changes to your breast following implantation are irreversible (cannot be undone) and breast implants may affect your ability to breastfeed, either by reducing or eliminating milk production. The most common complications with MemoryGel® breast implants include reoperation, capsular contracture, asymmetry, and breast pain. A lower risk of complication is implant rupture, which is most often silent (meaning neither you nor your doctor will know you have a rupture). The health consequences of a ruptured silicone gel-filled breast implant have not been fully established. MRI screenings are recommended three years after initial implant surgery and then every two years after to detect silent rupture.
